The VA Black Hills Health Care System (VA BHHCS) is located in the beautiful Black Hills of western South Dakota, where we are known for our blue skies, pine forests, rich history, moderate climate and friendly western hospitality. The VA BHHCS provide! s primary, surgical, behavioral health, community living cente! r (nursing home care), and domiciliary (residential rehabilitation treatment program - RRTP) care for Veterans residing in western South Dakota and portions of Nebraska, North Dakota, Wyoming and Montana. Care is delivered through the Fort Meade (near Sturgis) and Hot Springs VA Medical Centers, as well as through a number of community-based outpatient and rural outreach clinics. VA Black Hills is part of VA Midwest Health Care Network (VISN 23).
Note: The full performance level of this vacancy is GS 5. The actual grade at which an applicant may be selected for this vacancy is in the range of GS5 to GS6 . The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.

Background and-or Security Investigation required DUTIES:
This position is located in the outpatient clinic area PACT Teams in Hot Springs, SD. Incumbent is responsible for performing a full range of complex and non-complex patient care procedures specified in the Licensed Practical Nurse functional description for the grade assigned. The LPN will be responsible for rooming patients, identi! fying normal and abnormal patient conditions, implementing nursing orde! rs, and conducting nursing interventions as prescribed and within license. Prepares patients for procedures and assists with tests and examinations within the Specialty Clinic (orthopedics, podiatry,general surgery, ENT, optometry, ophthalmology, and urology). Documents all procedures and observation in the patient's electronic medical record.Instructs patients in sound health practices taking into account age, illness,physical limitations, culture and socioeconomic status. Performs routing administrative functions including ordering supplies and assuring each room isequipped and has materials needed for optimal patient flow. Applicants should have an interest in providing quality care for patients.
Work schedule: Monday - Friday, 7:30am -4:00pm

BASIC REQUIREMENTS:
U.S. citizenship.
Must have graduated from a school of practical or vocational nursing, approved by the appropriate State agency and/or accrediting agency and/or the National League for Nursing at the time the program was completed by the applicant.
Incumbent must possess and maintain a full, active, current, and unrestricted license as a graduate/licensed practical or vocational nurse in a State, Territory, or Commonwealth of the United States or District of Columbia.
Applicants will be expected to pass and maintain the physical requirements established for this position and are also subject to pre-employment and random drug testing.
Language Proficiency Requirement: Incumbents must be proficient in spoken and written English.
Grade Determinations:
In addition to the basic requirements stated in section A, the following qualification criteria must be met for each grade. The cand! idate's qualifications must clearly demonstrate the level of competence! required for the grade.
For GS-5: (a) Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent. (b) Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. (c) Demonstrated ability to serve as a responsible member of the nursing team and interact with patients, family members, professional and supportive personnel who provide patient care, and with members of other services.
For GS-6: In addition to meeting the requirements for the GS-5 level, has completed at least an additional year of qualifying experience at the GS-5 level or equivalent. Has the knowledge and skills necessary for concentrated effort and self-direction in carrying out the most complex practical nursing care assignments, including assignments for selected acute care, long-term or ambulatory care, or psychiatric patients requiring skilled nursing car! e. Serves as a member of a health care team with these selected patients and provides care to them on a regular and recurring basis. Completes assigned duties and responsibilities which involve performing nonstandard and specialized practical nursing activities at the GS-6 level, requiring broad work experience and demonstrated skill sufficient to resolve a range of nursing problems.
NOTE: Promotion to each level is dependent upon meeting criteria-based performance standards and grade level criteria set by LPN Professional Standards Board.

PHYSICAL REQUIREMENTS:
Position requires standing up to 4 hours, walking up to 4 hours, bending/stooping up to 2 hours, heavy lifting/carrying (45 pounds and over) and transferring in administration of patient care. In addition, the incumbent must be emotionally stable and adjust to different types of personalities, demonstrating cheerfulness, compassion, courtesy and concern. He/she must be quick and dependable in taking emergency action as directed and use initiative in preventing accidents.
