Position Summary
The Provider Relationship Associate is responsible for introducing the Healthmap Kidney Management program to physician practices. The primary focus of the position is to serve as a liaison between the physician practices and HealthMap’s Kidney Management program.

Position Summary
The Provider Relationship Associate is responsible for introducing the Healthmap Kidney Management program to physician practices. The primary focus of the position is to serve as a liaison between the physician practices and HealthMap’s Kidney Management program.
Responsibilities
- Collect and manage all provider profiles for up-to-date practice information
- Facilitate and schedule provider and practice onboarding meetings
- Monitor and manage key deliverables of the physician office implementation and onboarding processes
- Build strong cross-functional relationships with internal departments, physician partners, and practice staff
- Introduce kidney health program to office staff and physicians
- Work collaboratively with Clinical Practice Liaisons to support initiatives
- Ensure consistency of outreach and engagement with your book of business
- Ensure that all SLAs and department KPIs are met and/or exceeded
- Maintain and document all applicable practice and patient updates, follow up communications, and applicable dashboard information
- Establish all critical workflow processes for each assigned practice
- Perform other duties/tasks as assigned
- Associate degree or equivalent level of experience required. Bachelor’s degree preferred
- 2 years of experience in healthcare, managed care, physician office, or account management preferred
- Ability to problem-solve and execute initiatives
- Must have superb verbal and written communication skills
- Must possess a personal presence characterized by a sense of honesty, integrity, authenticity, confidence, and caring with the ability to portray the HealthMap solutions mission, vision, and goals
- Must possess the ability to close onboardings
- Ability to manage multiple priorities
- Must be proficient in Microsoft Office: Outlook, Word, Excel, PowerPoint
