The Clinical Care Coordin! ator (CCC) manages healthcare team members who provide individualized nursing care to adult and geriatric patients with cardiovascular disease undergoing procedural or surgical intervention. The CCC must have excellent communication skills to effectively counsel, support, and lead team members. Expected to lead others through change and crisis situations. Must have excellent critical thinking skills to problem solve issues. Must be able to delegate responsibilities to team members based on skill sets and scope of practice. Expected do complete annual reviews on team members. Some other responsibilities within role include: serve on committees within the department, manage resource issues (staffing), oversee scheduling conflicts and facilitate orientation and on-going education of employees. Expected to maintain the RN competencies of the HVA Department. As ambassador of the organization is expected to adhere to hospital, and unit philosophy, goals and standards of care. The! nurse utilizes independent and interdependent nursing judgmen! t when integrating health care. The nurse has knowledge of and utilizes appropriate age-related patient care protocols relating to the physical and psychological needs all clients according to department scope of service.

Following the RN Legal Scope of Practice, the CCC must be knowledgeable and competent in the care of various cardiovascular diagnoses, procedures and surgeries including, but not limited to: cardiac catheterization, pacemaker/AICD, EECP, tilt-table, cardio version, peripheral angiogram, venous closure, TEE, fistula insertion/declot, AAA, femoral/popliteal bypass, and CABG. The HVA CCC is expected to fulfill the following skills, but is not limited to: cardiac monitoring, arrhythmia interpretation and treatment, acute patient assessment and response, pre and post op cardiovascular procedural and surgical care. The CCC must be knowledgeable in the care of HVA patients. Must be able to interpret arrhythmias and treat correctly. Must have clinical com! petence to serve as resource to team members or other healthcare team members. Must be self-directed with the ability to lead others. Must be able to critically think quickly under stress and respond appropriately to crisis situations. Must be able to effectively communicate to physicians and other healthcare team members. Must be able to collaborate and negotiate with co-workers and other healthcare team members. Must be able to effectively manage conflict.

The RN functions within the scope and standards of nursing practice as outlined in the South Dakota Nurse Practice Act and Administrative Rules. The Sanford Nursing Practice recognizes the Scope and Standards of Practice (2004) and the Code of Ethics for Nurses with Interpretive Statements (2001) as published by the American Nurses Association as the foundation for nursing care delivery.

Required Skills

Bachelor's degree required. A minimum of 1 year of hands-on nursing experience required, 2 ! years of hands-on nursing experience is preferred. Demonstrated involve! ment in clinical process improvement initiatives preferred.

Currently licensed with the applicable State Nursing Board and/or possess multistate licensure privileges as required by position. Prefer certification in nursing specialty. Maintains required department specific competencies and certifications (i.e. - ACLS, PALS, TNCC, NRP, etc.). Functions within legal RN scope of practice. ACLS is required and must be obtain within 6 months of hire.
