The VA Black Hills Health Care System (VA BHHCS) is located in the beautiful Black Hills of western South Dakota, where we are known for our blue skies, pine forests, rich history, moderate climate and friendly western hospitality. The VA BHHCS provides primary, surgical, behavioral health, community living center (nursing home care), and domiciliary (residential rehabilitation treatment program - RRTP) care for Veterans residing in western South Dakota and portions of Nebraska, North Dakota, Wyoming and Montana. Care is delivered through the Fort Meade (near Sturgis) and Hot Springs VA Medical Centers, as well as through a number of community-based outpatient and rural outreach clinics. VA Black Hills is part of VA Midwest Health Care Network (VISN 23).
NOTE: Applicant's education and length of nursing practice! (experience) will be considered by a Nurse Professional Stand! ards Board in determining the grade and step of the applicant selected.

The Non VA Referral Case Manager (RCM) demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The RCM is responsible for providing advanced clinical and administrative r! ecommendations relative to programs, committees, and/or services that related to referral case management, Non VA care and utilization management for VA Black Hills Health Care System (VABHHCS), and includes the management of Non-VA care consults. The incumbent manages all VABHHCS Veteran patients in private hospitals, and monitors private care, level of complexity and care, and need for continued hospitalization. The incumbent evaluates when the patient is stable for discharge, or ready for transfer to the level of care provided at the VA medical centers located in Hot Springs and Fort Meade, SD. This involves daily review of inpatient medical records and consultation with case managers at local and regional hospitals. The RCM arranges for transfer back to VABHHCS, which includes collaboration with the unit Nurse Manager, Hospitalist and transfering facility. This position will be expected to complete rounds with outside facility case managers on Veterans in private hospit! als. This position will also be required to travel to all campus within! VABHHCS..
Interfacility transfers (Minneapolis/Omaha) are also coordinated by this position. Coordination of complex surgical procedure such as Cardiac and neurosurgery, this includes arranging for pre-operative work ups, travel and follow up care after surgery/procedure.
This position may include the additional role of transplant coordinator. This includes complete preparation, testing, coordination of support services, emotional support and patient education.
Location: This position is located in the Associate Director of Patient Care Services (ADPCS) Service Line at the Rapid City Clinic, Rapid City, SD.
Work Schedule: Monday through Friday 7:30am - 4:00pm

BASIC REQUIREMENTS:
U.S. citizenship.
Full unrestricted Licensure, Certification, or Registration required
Graduate of a schoo! l of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
Current, full, active, and unrestricted registrat! ion as a graduate professional nurse in a State, Territory or Commonweal! th (i.e., Puerto Rico) of the United States, or the District of Columbia.
EDUCATION/EXPERIENCE REQUIREMENTS:
At a minimum, applicants should possess experience as an RN that is determined to be sustained and consistently at or above an acceptable level of quality. Professional nursing experience should be documented on the Application for Employment - Nurse and Nurse Anesthetists (VA Form 10-2850a).
GRADE DETERMINATIONS:
Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.
